  • Generic tools copied from resource vendors.
  • Not adapted to the RTO’s cohort, industry context, or delivery mode.
  • Issues identified in pre-validation but never corrected.
  • No record of improvements made.

Missing Evidence

  • RTOs can’t produce pre-validation forms at audit.
